Santa Clara County is pleased to be planning the launch of its Office on Disability Affairs in Fall 2022. Through a collaboration with the World Institute on Disability, the County is now collecting feedback about how the Office can best prioritize the needs of people with disabilities who live, work, or receive services in the County.

If you have a disability or are a caretaker for a person with a disability, please visit www.wid.org/scc and take this survey to offer your feedback. Surveys are anonymous with an option to enter your contact information if you’d like to be included in a focus group to share further feedback (subject to maximum availability).

Surveys will be collected throughout the month of September. For additional information or to request an accessibility accommodation, please contact scc@wid.org.

Surveys are available in English, Spanish, Vietnamese, Chinese (Mandarin, I think), and Tagalog. Let me know if you need more information: michelem1029@gmail.com.
